Event Participant Liability Waiver
Effective 1/28/2024

WAIVER OF LIABILITY: In return for participating in the Communities Unite fundraiser event and related activities, the undersigned Participant or Parent / Legal Guardian of a participant under the age of 18, releases and agrees not to sue Communities Unite or any of its officers, Directors, Employees, Sub-Contractors, Sponsors, or Affiliates from all present and future claims that may be made by myself, my family, estate, or heirs for property damage, personal injury, or wrongful death arising from my participation.

 

I understand and agree Communities Unite is not responsible for injury or property damage arising from the event or activities caused by accident or my wrongdoing.

I understand that participation in the event/activities involves certain risks, including, but not limited to, serious injury. I am voluntarily participating in the fundraiser event with knowledge of any danger involved and I agree to accept all risks of participation.

 

I agree to indemnify and hold harmless Communities Unite for all claims arising from my participation in the event and any activity I decide to personally involve myself in.

 

I understand this document is intended to be as broad as permitted by the laws of the state of Pennsylvania in which this event is held. I acknowledge that Communities Unite has not arranged and does not carry any insurance of any kind for my benefit, persons under 18 years old, guardians, heirs, or successors. I represent to my knowledge, am in good health and suffer no physical impairment that would prevent my participation in this event held by Communities Unite for the charity fundraiser event.

 

I understand this agreement grants certain privileges to and eliminates the liability of Communities Unite.

I have read this form and am of legal age to freely agree to these terms. By agreeing, I am giving up my legal rights and remedies.

 

Parent / Legal Guardian if person a is under 18

I am the parent or legal guardian of the Participant. I am of legal age and freely sign this agreement. I have read this document and understand that by signing, I am giving up legal rights and remedies.
